Home Health Occupational Therapist/Contractor

We are looking for a per diem Occupational Therapist with recent Home Health experience contractor or employee to visit patients in their homes in the Riverside and Moreno Valley area.

Responsibilities and Duties:
- Assess patient's condition under a physicians orders during every visit and chart observations
- Perform evaluations, Discharges, Recerts and possible follow up visits
- Follow prescribed treatments, following plan of care and assess progress of healing
- Provide appropriate timely feedback to physician and other professionals
- Charting and documenting on EMR system
Compensation: $75.00 - $110.00 per hour
